Our office will make every effort to schedule your patients to be seen by one of our providers as soon as possible.
It’s our office policy to mail out a New Patient Packet to every new patient. However, if you would like some of
our Packets on hand, please feel free to call our office and we will provide them for your staff. Packets are
available in English and Spanish.

Please call our office for the passwords to open these documents.
With a written referral from your office, all patients are expected to show-up for their first appointment with their
completed new patient questionnaire and any recent X-rays or MRI films. This is stressed to the patient several
times, written and verbal. Regretfully, if a patient shows up for his or her appointment without the paperwork
completed, it will result in re-scheduling. Our office will call the patient approximately two days before their
appointment for that friendly reminder.
No matter which office your patient is scheduled to see one of our providers, kindly mail or fax medical records pertinent to our services directly to the main office (listed below).
Dictated reports are done for all our patients. Our staff will mail or fax reports directly to the office you request.
Turn around time is normally 1 to 2 days.
