RE: IME’s
(Independent Medical Examinations)
Dear Representative:
Let us take this opportunity to Thank
You for the IME referrals you have sent our way.
The doctors and staff of the Comprehensive Pain Management Group
are happy to work with you and to accommodate your needs.
Kindly take some time to review our IME policy and please feel free
to contact our office directly for any clarifications.
IME Policy:
- All
IME’s are to be scheduled with our Medical Director, Howard Konowitz,
MD
- All
patients will receive a New Patient Packet with an Initial Pain
Questionnaire. The patient must arrive for their appointment with the
Questionnaire complete.
- A
patient who doesn’t receive a New Patient Packet prior to their
appointment must arrive (1) one hour before his/hers scheduled
appointment to complete their paperwork.
- If
the patient refuses to fill out the Initial Pain Questionnaire, the
physician will not be able to perform an examination and the visit
will be terminated.
- If
the patient does not keep his/hers scheduled appointment without
proper notification, or the visit has to be terminated due to the
refusal or untimely completion of the Initial Pain Questionnaire, a
fee will be billed to the party who scheduled the IME visit.
- IME
visits can only be cancelled and/or rescheduled by the party who
scheduled the initial visit. We will not accept the patient’s
cancellation and bill for a No Show visit.
- All
medical data needed for the IME is to be mailed to our Main
Office,

. This is where our
charts are prepared and maintained.
- Mailing
or faxing medical data to any other office or fax number may result in
the IME physician not receiving critical information to complete your
IME request, or possible cancellation of the IME.
- Please
do not mail x-ray or imaging films to our office rather have the
patient bring them to the appointment themselves.
